Forms are required for Federal Student Aid accountability. Documents are scanned and originals are retained in the office for one year, then archived according to federal and state retention policies. Submit only 2014-15 forms, and submit only forms that are requested. Check your Butte College email and MyBC for "Required Documents." Forms below are arranged in alphabetical order for your convenience.


14-15 ACAD PROGRAM . Your Butte College account has been identified as having an UNDECLARED Academic Program. Please update your Academic Program with Admissions and Records.

INSTRUCTIONS:Please visit any Admissions and Records location or call 530-895-2511 for assistance in updating your Academic Program. Select an eligible Academic Program available for your Education Goal (Associate's Degree or Certificate of Achievement at Butte College). Once your Academic Program has been updated, please contact the Financial Aid Office.


CAL GRANT H.S. GRADUATION VERIFICATION. Necessary to receive the Cal Grant and all other financial aid for which you may qualify. AFTER you graduate from high school, you must verify your HS grad date on the CSAC website at https://mygrantinfo.csac.ca.gov/logon.asp


CHILD SUPPORT VERIFICATION. For students required to verify either their or their spouse's child support PAID, or their parent(s)' child support PAID.


DECLINE PELL GRANT FORM. Pell Grant is limited to 6 full-time years or 600% Lifetime Eligibility Used. This form is only for students who choose NOT to receive their scheduled Pell award for 2014-2015. See form for instructions. 


14-15 EDUCATION GOAL . Your Butte College account has been identified as having an UNDECIDED Education Goal.

IMPORTANT: Eligible Education Goals for financial aid are Transfer with AA/AS; Transfer without AA/AS; AA/AS without Transfer; Vocational Degree without Transfer; or Vocational Certificate of Achievement without Transfer.

INSTRUCTIONS: Please visit any Admissions and Records location or call 530-895-2511 for assistance in updating your Educational Goal. Once your Educational Goal is updated, please contact the Financial Aid Office.


ELIGIBLE NON-CITIZEN VERIFICATION. Required for students who are eligible non-citizens to document their non-citizen eligibility for Federal Student Aid programs.


FAFSA DEPENDENCY STATUS VERIFICATION FORM. Required for students, who answered "yes" on the FAFSA to one of the following:


FAFSA REJECTED. Your 2014-2015 Free Application for Federal Student Aid was rejected by the federal processor.

Instructions: As a Returning User, go to the Federal Student Aid Web Site at www.fafsa.ed.gov. Click on "Login", then select your Student Aid Report (SAR) to view.

Go to the section entitled "FAA Information". There you will find the reason(s) your FAFSA was rejected and what actions you must take. For additional information, read the section called "Comments" from the left navigation bar. Note: The resolution of a rejected FAFSA is the responsibility of the student.


 HIGH SCHOOL COMPLETION STATUS VERIFICATION SHEET. Instructions for students required to document their high school completion status for the 2014-2015 academic year. This is a Federal verification form only. For Cal Grant high school verification, see above for Cal Grant High School Verification.


HOUSEHOLD SIZE AND NUMBER IN COLLEGE VERIFICATION. For students required to verify their household size and number in college.


IDENTITY AND STATEMENT OF EDUCATIONAL PURPOSE: INSTITUTIONAL Verification. The student MUST PRESENT a valid government-issued photo ID and sign this form IN PERSON at the Financial Aid Office. If student cannot appear in person, student should submit NOTARY version below.


IDENTITY AND STATEMENT OF EDUCATIONAL PURPOSE: NOTARY Verification. This form MUST  PRESENT a valid government-issued photo ID, and sign IN PERSON before a NOTARY. The original form must then be mailed (no FAX or email). See form for instructions.


INCOME TAX RETURN INFORMATION SHEET FOR SPECIAL FILING CIRCUMSTANCES. Special filing circumstances: (1) IRS filing extensions; (2) amended IRS income tax returns; (3) tax filers who were victims of IRS identity theft; and (4) filers of Non-IRS income tax returns.


LOAN DEFAULT. You are not eligible to receive any federal student aid until your default has been resolved.

Federal regulations require that you make satisfactory arrangements with the servicer of your loan to repay the defaulted loan. Once you have repaid the loan or made satisfactory arrangements to repay the loan, please submit documentation to our office and we will reactivate your application for financial aid. Go to www.myeddebt.com for information and assistance or call 1-800-621-3115 for questions regarding your defaulted student loan account.


ONE-SEMESTER LOAN REQUEST. Form required for students who indicated on their Direct Loan application that their loan request is for Fall 2014 only.


PARENT INCOME TAX FILER VERIFICATION. For students whose parent(s) are required to provide filed IRS income tax information for 2013. Print and submit Parent Income Tax Filer Verification form and provide tax information by one of the following:

(1) Use the IRS Data Retrieval Tool(VIDEO) to provide tax information (preferred); or (2) Order a 2013 Tax Return Transcript at www.irs.gov and attach to the Parent Income Tax Filer Verification form. 


PARENT NON-TAX FILER INCOME VERIFICATION. For students whose parent(s) are required to provide verification that they have not filed, will not file, and ARE NOT REQUIRED to file a 2013 federal tax return. See form for instructions.


RECEIPT OF SNAP BENEFITS VERIFICATION. For students required to verify either their own, their spouse's,  or their parent(s)' receipt of SNAP benefits in 2012 or 2013. 


SELECTIVE SERVICE REGISTRATION. You must submit proof of Selective Service registration before you can receive federal student aid. Go to www.sss.gov to register and print out confirmation of registration.

If you are over the age of 26 and never registered, you will have to print Request for Status Information Letter from the Selective Service System website. Follow instructions to complete the request form and mail it to Selective Service System for evaluation of your information.  Once you have received your response letter from Selective Service, you will need to submit it to our office with a typed letter as to why you did not register.

14-15 UNDECL/OLD PROGRAMS.

Your Butte College account has been identified as having an UNDECIDED Education Goal and an UNDECLARED Academic Program. Please update both your Education Goal and Academic Program with Admissions and Records.

INSTRUCTIONS: Please visit any Admissions and Records location or call 530-895-2511 for assistance in updating your Educational Goal and/or Academic Program. Once your Educational Goal and/or Academic Program  has been updated, please contact the Financial Aid Office.


UNTAXED INCOME VERIFICATION. For students and parents required to verify untaxed income. See form for instructions.


 14-15 Appeal Forms


COURSE EVALUATION FOR FINANCIAL AID. For students who are requested to submit a Course Evaluation for an Excess Units Petition, Loan Application or Satisfactory Academic Progress Appeal.


2014-2015 DEPENDENCY STATUS APPEAL (INITIAL REQUEST). For applicants with adverse home situations, to request review of their Dependent status for consideration as Independent for financial aid need analysis. Follow all instructions.

2015-2016 DEPENDENCY STATUS APPEAL: COMING SOON-Submit this form if you are planning to attend the 2015 Fall semester/2016 Spring semester.


2014-2015 DEPENDENCY OVERRIDE RENEWAL. For students who were previously granted a Dependency Override by Butte College. Students with initial requests must use the Dependency Status Appeal (see above). See form for instructions. 

2015-2016 DEPENDENCY OVERRIDE RENEWAL: COMING SOON-Submit this form if you are planning to attend the 2015 Fall semester/2016 Spring semester.


EXCESS UNITS PETITION. For students disqualified for exceeding SAP time frame criteria, and who wish to appeal to extend their eligibility. Deadline for submitting petition: October 30 for Fall; March 30 for Spring. See form for instructions. 


FINANCIAL AID PROBATION CONTRACT. For students whose SAP Appeal for Extenuating Circumstances has been approved, or for students approved for Probation due to term improvement. See form for instructions. 


PARENT APPEAL FOR UNUSUAL CIRCUMSTANCES. Available January 5th. For parents of applicants to request adjustments to their student's federal student aid application based on unusual circumstances. See form for instructions.


SATISFACTORY ACADEMIC PROGRESS EXTENUATING CIRCUMSTANCES APPEAL. For students with extenuating circumstances who failed to meet SAP criteria. Deadline for submitting appeal: October 30 for Fall; March 30 for Spring. See form for instructions.


STUDENT APPEAL FOR UNUSUAL CIRCUMSTANCES. Available January 5th. For students to request adjustments to their federal student aid application based on unusual circumstances. See form for instructions.
