Forms are required for Federal Student Aid accountability. Documents are scanned and retained in the office for one year, then archived according to federal and state retention Policies. 

Guidelines for submitting forms:

Application for Summer 2014 Pell Grant. Students must register for Summer Session before completing this application. Summer Session Pell Grant may be available for students who: (1) have a 2013-2014 FAFSA on file with our office; (2) have been determined to be eligible for Pell and have remaining 2013-2014 eligibility; and (3) require completion of Summer courses for their Academic Program. NOTE: Pell will be adjusted according to eligibility, as determined by enrollment and need. Full-time enrollment is considered to be 12 units.

IMPORTANT: Student should apply for the 2014-2015 FAFSA to receive the BOGFW to cover summer units fees.                                                                                                                             

Child Support VerificationFor students required to verify either their or their spouse's child support PAID, or their parent(s)' child support PAID.

Eligible Non-Citizen Verification. Required for students who are eligible non-citizens to document their non-citizen eligibility for Federal Student Aid programs.

FAFSA Dependency Status Verification Form. Required for students, who answered "yes" on the FAFSA to being orphaned, in foster care, a ward of the court, an emancipated minor, in legal guardianship, or homeless, to document their status.

High School Completion Status Verification Sheet. Instructions for students required to document their high school completion status for the 2013-2014 academic year.

Household Size and Number in College Verification. For students required to verify their household size and number in college.

Identity and Statement of Educational Purpose: INSTITUTIONAL Verification. For students required to verify their identity and educational purpose. The student MUST PRESENT a valid government-issued photo ID and sign this form IN PERSON at the Financial Aid Office. If student cannot appear in person, student should submit NOTARY version below.

Identity and Statement of Educational Purpose: NOTARY Verification. For students required to verify their identity and educational purpose. This form MUST BE PRESENTED, along with a valid government-issued photo ID, to a public NOTARY. The original form must then be mailed (no FAX or email) to Butte College Financial Aid Office, 3536 Butte Campus Drive, Oroville CA 95965.

Income Tax Return Information Sheet for Special Filing Circumstances. Instructions for special filing circumstances: (1) filing extensions granted by the IRS; (2) amended IRS income tax returns; (3) tax filers who were victims of IRS identity theft; and (4) individuals who filed Non-IRS income tax returns.

Parent Income Tax Filer Verification. For students whose parent(s) are required to provide filed IRS income tax information for 2012.  This form must be submitted in addition to doing one of the following: (1) Use the IRS Data Retrieval Tool(VIDEO) to provide tax information (preferred); or (2) Order a 2012 Tax Return Transcript at www.irs.gov and attach to the Parent Income Tax Filer Verification form. Click for sample of a Tax Return Transcript.

Parent Non-Tax Filer Income Verification. For students whose parent(s) are required to provide verification that they have not filed, will not file, and ARE NOT REQUIRED to file a 2012 federal tax return. This form must be completed in full and submitted with W-2 or 1099 statements issued to the parents.

Receipt of SNAP Benefits Verification. For students required to verify either their or spouse's receipt of SNAP benefits in 2011 or 2012, or their parent(s)' receipt of SNAP benefits in 2011 or 2012.

Student Income Tax Filer Verification. For student and spouse (if married) required to provide filed IRS income tax information for 2012. This form must be submitted in addition to doing one of the following:(1) Use the IRS Data Retrieval Tool(VIDEO) to provide tax information (preferred); or (2) Order 2012 Tax Return Transcript at www.irs.gov and attach to the Student Income Tax Filer Verification form.  13-14 Appeal Forms


Course Evaluation for Financial Aid. For students who are requested to submit a Course Evaluation for an Excess Units Petition, Loan Application or Satisfactory Academic Progress Appeal.

Dependency Status Appeal (INITIAL REQUEST). For applicants with adverse home situations, to request review of their Dependent status for consideration as Independent for financial aid need analysis. Click here for 2014-2015 Dependency Status Appeal.

Dependency Override RENEWAL. For students who were previously granted a Dependency Override by Butte College. Students with initial requests must use the Dependency Status Appeal (see above) and provide documentation as requested on the form. Click here for 2014-2015 Dependency Override RENEWAL.

Excess Units Petition. For students who failed to meet SAP time frame criteria. Students disqualified for exceeding time frame requirements may appeal by submitting an Excess Units Petition. Deadline for submitting petition: October 30 for Fall; March 30 for Spring.

Financial Aid Probation Contract. For students notified their SAP Appeal for Extenuating Circumstances has been approved, or for students approved for Probation due to term improvement. In order to receive financial aid while on Probation, students must return this signed form as soon as possible.

Parent Appeal for Unusual Circumstances. For parents of applicants who would like to request adjustments to their student’s federal student aid application based on unusual circumstances. Through an appeal process, we will determine if the circumstances will result in a change to student’s aid eligibility.

Satisfactory Academic Progress Extenuating Circumstances Appeal. For students with extenuating circumstances who failed to meet SAP criteria. See Appeal form for definition of extenuating circumstances. Appeals are for one-term only. Beginning in March, all appeals must include Course Evaluation (attached). Deadline to submit spring term appeal: April 30, 2014

Student Appeal for Unusual Circumstances. For students who would like to request adjustments to their federal student aid application based on unusual circumstances. Through an appeal process, we will determine if the circumstances will result in a change to your aid eligibility.


Other Forms


Decline Pell Grant Form. Pell Grant is limited to 6 full-time years or 600% Lifetime Eligibility Used. This form is only for students who choose NOT to receive their scheduled Pell award for 2013-2014. Please give careful consideration to the choice you make.

One-Semester Loan Request. Form required for students who indicated on their Direct Loan application that their loan request is for Fall 2013 only.
