Registration

You will only need to register once per semester. There is one mandatory form. (It can be downloaded, printed and then completed) Unfortunately, if it is not filled out, you will not be permitted to attend any training. Completing this form allows us to provide FREE training to YOU!

A Brief Explanation of MANDATORY Form:

1) 2012-2013 Participant Registration Form:

Download and print a copy of the 2012-2013 Participant Registration Form for the Foster and Kinship Care Education Program. Please fill it out completely. This mandatory form is very important; it is how we receive funding, which makes trainings such as these FREE to you!!

Please return the completed form to the office staff when arriving for the training. Be sure to indicate the name and date of the training you wish to attend. You can also mail the forms to us at: 2480 Notre Dame Blvd. Chico, CA. 95928. Or you may fax them to us at 530-897-6242.
